The following forms must be completed and turned into Oaktree with $65.00 application fee (per family) before children will be able to attend.

1. Download the Oaktree Registration packet
2. Fill out all the forms, signing where indicated.
3. Bring them into the Oaktree Office with you.
4. There will be two small index cards for you to fill out in the Oaktree office.
